Practice Hours
Consultations with the GPs are by appointment Monday to Friday 8.30am — 5.00pm
The practice is closed on Weekends and Public Holidays
Services available
Health assessments, family planning, Pap smears, pregnancy tests, ECGS, blood pressure monitoring, blood sugar levels (BSL’s), chronic disease management, childhood vaccinations, skin cancer checks, wound dressings, Asthma, Diabetes and Women’s Health Clinics.
Additional Services
Eye Health, Men’s Sexual Health, Mental Health, Podiatry, Transport, Drug and Alchohol program, Bringing Them Home program, Youth Program, and School Screening.
Fees and billing arrangements:
All consultations are bulk-billed at this Practice. Veterans will be required to sign the service voucher.
To make appointments:
Please ring 6845 3545 for an appointment. Every effort will be made to accommodate your preferred time and GP. Emergencies will always be given priority and our reception staff will attempt to contact you if there is any unforeseen delay or your GP has been called away. Longer consultation times are available, so please ask our receptionists if you require some extra time. If you or a family member require an interpreter service, we can organise this for you. Please let us know when you make the appointment.
Care outside normal opening hours:
The practice participates in 24 hour care for patients via a roster system at
Wellington
District
Hospital . The roster is shared with other Wellington GPs. The hospital is located on the corner of Gisborne and Thornton Sts.
Please ring the hospital number 6840 7200 or 000 in an emergency.
Home visits
Home visits are not routinely available for patients within this practice. However, you may discuss individual arrangements and circumstances with your doctor. Some visits are available from the Aboriginal Health Workers if required.
Telephone Access:
GPs in the practice may be contacted during normal surgery hours. If the GP is with patient, a message will be taken and the reception staff will advise you when it is likely that the GP will return your call. Your call will always be put through to the GP in an emergency
Management of your Personal Health Information:
Your medical record is a confidential document. It is the policy of this practice to maintain security of personal health information at all times and to ensure that this information is only available to authorised members of staff. We abide by the ten National Privacy Principles available at http://www,privacy.gov.au/health/index.html
Reminder & Recall System:
Our practice is committed to preventative care. You may be included on our reminder system. We may issue you with a reminder notice from time to time offering you preventative health services appropriate to your care. If you do not wish to be part of this system please let your doctor or reception staff know. Immunisation information will be passed onto the Australian Childhood Immunisation Register (ACIR) at a National level. Information will also be passed onto the NSW Pap Register as appropriate.
Updating your details:
If you have not been into the practice for a while we may need to update your details on our computer system. The reason for maintaining updated information is very important in order for us to provide you with a high standard of care. To ensure that we are in a position to provide the best and most appropriate services and referrals for you or your family we require the following information from you at reception:
Your full name
Your date of birth
Your current contact details including address, phone number, contact in case of an emergency
Whether you are of Aboriginal or Torres Strait Islander origin
Your current Medicare and/or health care card numbers
When you make your next visit please take the time to provide reception staff with your updated details so that your medical records remain current.
